This 12-week program includes regular one-on-one sessions with our dietitian and diabetes educator that specialise in diabetes management. As we all know that everyone is unique and different, so are our dietary needs. It is especially important to have personalised dietary and medical support from health professionals for someone who has a medical condition that require regular health monitoring.
We strive to support you holistically as a whole person for optimal health, not just to help manage your blood sugar. We will perform a thorough assessment on your blood test report, medical history, weight history, social history, eating habits and food preferences. We also assess other areas that may have an impact on your health such as your hormonal health and gut health as well as stress and other lifestyle factors before developing a personalised nutrition care plan tailored to your personal needs and preferences.
If you have a regular GP who looks after your health and diabetes, our dietitian and diabetes educator team will be happy to work closely with your GP and other health professionals as a team to provide the best holistic care and support. We will liaise with your GP and other health team involved throughout the program, including sending them reports and updates on your health progress and will advise you when to see your GP regarding medication adjustment during and after the program. Rest assure that our team will be there to support you all the way to provide you the best health outcomes possible that you desire.

By combining the dietitian’s expertise in nutrition with the diabetes educator’s focus on holistic diabetes management, you will receive a well-rounded, transformative experience. This collaborative approach ensures they are equipped with the tools, knowledge, and confidence to thrive far beyond the 12-week program.
At the end of the program, you will be required to see your GP to do a 3-monthly blood test for HbA1c to monitor your health progress and improvements on diabetes control. Your GP and/or your endocrinologist will receive an summary health report from us so that they can further assist you to work towards diabetes remission.
