We provide in-person, Telehealth (video or phone), home visits, and Residential Aged Care Facility (RACF) visits across locations in Brisbane and Gold Coast.

 

Referrals forms accepted (Click to download respective form/information:

- Enhanced Primary Care Plan (EPC) (Up to 5 sessions per calendar year)

- Eating Disorder Care Plan (EDP) (Up to 20 sessions per calendar year)
​​

- Patients under Referral for Allied Health Services under Medicare for People of Aboriginal or Torres Strait Islander descent (Up to 10 sessions per calendar year combined with EPC)
​​

- DVA referral form (D904)​​

- NDIS form​​

- T2DM group program (Up to 8 group sessions a year with Medicare rebates, $34.65 out of pocket per session)​​
